Abstract

Stroke is an acute brain functional disorder caused by obstruction of cerebral blood flow due to bleeding (hemorrhagic) or blockage (ischemia) which can lead to death or permanent disability. Based on WHO 13.7 million people have had a stroke and around 80 million people are living in a post-stroke condition [12]. Patients with stroke need comprehensive management starting from physical, psychological aspects, restoring roles and functions in life, and adequate support. The success of recovery in all aspects of stroke patients can be influenced by the quality of discharge planning from health personnel, so that it is expected to provide good readiness, especially for patients undergoing post-stroke maintenance. This study aims to determine the description of the patient^s readiness level in facing discharge in room X RSUD dr. Soekardjo Tasikmalaya. This research is a descriptive study with a sample of 27 people. The results showed that Personal Status was categorized as Level 1 (R1), Knowledge was categorized as Level 1 (R1), Coping Ability was categorized as Level 2 (R2), Expected Support was categorized as Level 2 (R2). Based on these results, it is suggested that the nurse should carry out discharge planning in a gradual and more structured manner to stroke patients in that place.
